29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.  a) Mold Room/Disa Department, Pour Deck: On or before November 22, 2019,  employees were not protected from fall hazards in that  barriers, used to prevent employees from falling from platforms, stepping into the tough when manually pouring molten metal from the ladle, taking temperature readings, removing slag or adding chemicals  (etc.) were not installed or utilized.
