29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.   a) On or about April 11, 2019, the employer did not document that equipment complied with its chosen recognized and generally accepted good engineering practices (RAGAGEP), such as IIAR 2 (2014), Standard for Safe Design of Closed Circuit Ammonia Refrigeration System, Section 5.14.5, when it did not label the liquid and ammonia vapor process piping located at air handling units 1 and 2.  b) On or about April 11, 2019, the employer did not document that equipment complied with its chosen recognized and generally accepted good engineering practices (RAGAGEP), such as IIAR 2 (2014) Standard for Safe Design of Closed Circuit Ammonia Refrigeration System, Section 5.14.5, when it did not label the high pressure ammonia discharge piping on reciprocating compressors 4 and 5.  c) On or about April 11, 2019, the employer did not document that equipment complied with its chosen recognized and generally accepted good engineering practices (RAGAGEP), such as IIAR 2 (2014) Standard for Safe Design of Closed Circuit Ammonia Refrigeration System, Section 6.7.1 when it did have access to a minimum of two eyewash/safety shower units, one located inside the machinery room and one located outside of the machinery room.   Abatement certification must be submitted for this item.

In addition, the emergency action plan did not include procedures for handling small releases:   a) At the facility, on or about 4/11/2019: The employers written emergency action plan did not include accurate procedures for the handling of small releases: 1) The written action plan defined a small release as airborne concentrations less than 250 ppm, while persons on site said they would not respond to releases exceeding 25 ppm 2) The written action plan mentioned the use of air purifying respirators while investigating releases and there were no air purifying respirators on site 3) Emergency escape respirators were located inside of the engine room, the written plan made no mention of the use of escape respirators.    Abatement certification must be submitted for this item.
