On or about April 16, 2019 employees were exposed to fall hazards from heights up to 4 feet, 2.5 inches where the employer provided a refrigerator for employee use and employees maintained an improvised break area adjacent to shipping dock doors on the north side of the production area near the trim saw station and the employer did not provide fall protection meeting the requirements of this standard.
