Employees were in the process of installing water pipes while standing on the steering arm of the trucks, on or about April 17,2019, an employee was stepping down, the right foot became tangled in a wiring harness, as result the employee fell backwards onto head and shoulder.

Employees were in the process of installing water pipes for radiators, the employees were standing on the truck frames that is approximately three and half ft. above the concrete floor.  On or about April 17,2019, as an employee stepped down the right foot became tangled in a wiring harness, this resulted in a fall and striking the head against of the concrete floor.
