29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition:    (a)  On or about April 15, 2019, the employer did not inspect working surfaces, such as the dock plates at doors 41, 32, 23 and 13, to ensure that deficiencies in the surfaces, such as the broken concrete of the dock at the edge of the steel plate or the 2.5 inch gaps between the dock plates and dock plate bumpers at each side of the doorway, would not cause injuries to employees who were crossing these dock plates with fork lifts while loading and unloading freight from trailers as part of their daily job duties.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness.    On or about May 1, 2019, at USF Holland, LLC, Dayton, OH, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out completely or correctly for the calendar year 2019.    (a)  On or about April 15, 2019, an employee was hospitalized when they suffered chest wall contusions and chest pain from a powered industrial vehicle accident on a broken dock plate.  The employer did not complete the Safety Risk Online Injury Report (equivalent form to OSHA 301) with details of the hospitalization and information about the physician or other healthcare professional.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    (a)  On or about April 15, 2019, an employee was injured when the forklift they were driving hit a broken dock plate when entering a trailer to pick up freight.  The employee was hospitalized for these injuries on April 15 and released on April 18, 2019, but the employer did not report the hospitalization to OSHA as required.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
