Five Star Highland Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1229981, citation 1, item 1, and was affirmed final order on July 6, 2017, with respect to a workplace located at 16126 Syd Creek Dr., Lockport, IL 60491. Five Star Highland Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1161817, citation 1, item 1, and was affirmed final order on August 9, 2016, with respect to a workplace located at 259 Holmes St., Clarendon Hills, IL 60514. Five Star Highland Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1149777, citation 1, item 2, and was affirmed final order on July 20, 2016, with respect to a workplace located at 627 N. Rolling Ln., Arlington Heights, IL 60005. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
