29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one stairrail system along each unprotected side or edge:    a)  On May 3, 2019, employees were accessing the upper floors of the building by way of stairways that did not have a stairrail system along the unprotected side. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On May 3, 2019, employees performing carpentry work were exposed to fall hazards of approximately 17 feet.    Five Star Highland Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1229981, citation 1, item 1, and was affirmed final order on July 6, 2017, with respect to a workplace located at 16126 Syd Creek Dr., Lockport, IL 60491.      Five Star Highland Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1161817, citation 1, item 1, and was affirmed final order on August 9, 2016, with respect to a workplace located at 259 Holmes St., Clarendon Hills, IL 60514.      Five Star Highland Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1149777, citation 1, item 2, and was affirmed final order on July 20, 2016, with respect to a workplace located at 627 N. Rolling Ln., Arlington Heights, IL 60005.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
