29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a) On or about May 6, 2019; north-side of the residential home (front); employees did not have fall protection in place while working on a ladder-jack scaffold system while installing gutters exposing them to a fall hazard of 18-feet to ground level.    No abatement certification or documentation required for this item.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:  a) On or about May 6, 2019; front of building' employees working on the ladder-jack scaffold system were not aware of the requirements and/or limitations such as but not limited to the maximum height in which the equipment can be used.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  a) On or about May 6, 2019; front area of the residential home (north-side); employees were using an extension ladder that the rails were extended 6-inches above the working platform exposing them to a fall hazard of approximately 18-feet to ground level.  No abatement certification or documentation required for this item.
