All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about April 19, 2019 prior to performing setup activities on the Borghi U-Twist machine, the hazardous energy was not isolated and/or equivalent measures were not taken to ensure employees were protected against pinch point and/or crushing hazards.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: On or about April 20, 2019 the employer had not reported a work-related incident that occurred on April 19, 2019, which resulted in an employee suffering an amputation injury.
