29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On December 18th 2018, at a jobsite located at 8410 Euclid Avenue, Kansas City, Missouri 64132, employees working near the trash trailer were exposed to serious struck by injuries, in that they were not wearing a protective helmet to protect their heads from potential falling roofing debris being thrown into the trailer from the roof above.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On December 18th 2018, at a job site located at 8410 Euclid Avenue, Kansas City, Missouri 64132, employees were exposed to serious injuries while conducting residential construction activities, in that the employees were not provided with a guardrail system, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure. The employees were exposed to an approximate 8 foot 6 inch fall hazard to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. On December 18th 2018 at worksite 8410 Euclid Avenue, Kansas City, Missouri 64132, Employees were accessing a working surface on a residential roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.
