29 CFR 1910.29(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guiderail system and the walking-working surface.    On 4/29/19, employees were exposed to trip and fall hazards as in areas of the facilities fall hazards were protected by a single upper chain rail only and not an upper and a lower guardrail. Instances included:    a - Single chain rail at Final Effluent Flume  at Cedar Creek Facility  b - Single chain rails around all water treatment ponds  at Water Treatment Facility  c - Gap of guardrails at entry door to building - at Water Treatment Facility  d - at the Grit Classifier  at Marburg Facility   e - at Hydrotech Disc Filter  at Marburg Facility
