29 CFR 1910.22(a)(1): Places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.  ESG Operations, Inc., Cumming, Ga - Employees walking on the side walk into the waste treatment area were exposed to tripping hazards to include, but not limited to vent pipes and manhole covers creating a tripping hazard, on or about 7/30/19.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  ESG Operations, Inc., Cumming, Ga - At the waste water plant, the emergency eye wash did not have a shower available in the event of an emergency splash of potassium permanganate to the body, on or about 07/30/19.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    ESG Operations, Inc., Cumming, Ga - At the waste water plant, plant operators handle corrosive chemicals such as but not limited to potassium permanganate. The use of chemical resistant goggles was not ensured, on or about 07/30/19.    ESG Operations, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.133(a)(1), which was contained in OSHA inspection number 1397847, citation number 1, item number 1 and was affirmed as a final order on 07/18/19, with respect to a workplace located at 702 Creekside Lane, Winder, GA.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  ESG Operations, Inc., Cumming, Ga - At the waste water plants, plant operators handle corrosive chemicals such as but not limited to potassium permanganate. The employer's PPE assessment did not include an assessment of the hazards of potassium permanganate such as of face shield and chemical resistant clothing, on or about 07/30/19. Potassium permanganate is a newly added chemical to the process.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
