29 CFR 1926.651(b)(3): When the excavation operations approach the estimated location of underground installations, the employer did not determine the exact location of the installations by a safe and acceptable means: (a) Gilbert Contracting Inc. at 469 West Highway 7, Broomfield, CO 80023: On and before May 6, 2019 the employer did not determine the exact location or use a safe and acceptable means to find underground utilities in that, an employee using an excavator struck an underground electrical utility. The ruptured electrical lines caused a power outage to a building under construction at the site. This condition exposed employees to potential electric shock and/or electrocution hazards.
