Employer failed to ensure workers were working in a trench where the trench box 's bottom edge was no more than two feet from the bottom of the trench.

29 CFR  1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  a) 9517 Ralston Road to 59th ave (near Brooksdale Meridian Senior Home) in Arvada, CO 80001: On or about October 20, 2022, employees were exposed to cave-in hazards in that an inspection of the trench was not conducted by a competent person. Additional hand digging in the trench increased the depth of the trench below the box beyond two feet. Employer failed to ensure a competent person was trained on recognizing these types of hazards.
