29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Winch trailer - On or about May 16, 2019 the winch being used to lift tools and equipment to employees working on the tower did not have strain relief on the main power cord and hoist controls, exposing employees to electrical hazards.

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: (a) Communication tower - On or about May 17, 2019 the employer did not ensure an employee was utilizing a means of fall protection while performing work on a communications tower exposing the employee to a fall hazard.
