On or about August 27, 2019 at 13-16 133rd Place, College Point, NY 11356 (Shop Area) The employer failed to ensure a metal bending press brake used by employees was properly guarded at the point of operation exposing employees to amputation hazards. The hazardous condition resulted in an amputation that occurred on August 27, 2019. ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
