On or about May 20, 2019, at a job-site located at 2715 Easton St NE, Canton, Ohio 44721; the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with near vertical side walls and a depth of approximately 12.5 feet.  The trench was determined to have type "C" soil.
