29 CFR 1960.8(a): The head of the Agency did not furnish to each employee employment and a place of employment which was free from recognized hazards that were causing or were likely to cause death or serious physical harm, in that employees were exposed to struck-by and crush hazards: a) Warehouse, Building 376 - On or about O5/21/19 and at times prior thereto employees worked around roller conveyors operating in intermittent mode, and were exposed to unprotected ingoing nip points created by the rollers, the conveyor band, and the fixed parts of the structure. The employer did not ensure that all sections of the conveyor system were protected. b) Warehouse, Building 376 - On or about 05/21/19 and at times prior thereto employees worked around multi-track belt conveyors operating in intermittent mode, and were exposed to unprotected ingoing nip points created by the rollers, the conveyor band, and the fixed parts of the structure. The employer did not ensure that all sections of the conveyor system were protected. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, and flying chips: a) Warehouse, Building #351 - On or about 05/21/19 and at times prior thereto, the power feed conveyor (inlet), the blade, and the outlet section of the conveyor in the Rip Saw #OW4330 (Mattison Machine Works, Model #404, S/N 23282) were not guarded. b) Warehouse, Building #351 - On or about 05/21/19 and at times prior thereto, the power feed conveyor (inlet), the blade, and the outlet section of the conveyor in the Rip Saw #OW4329 (Mattison Machine Works, Model #404, S/N 22920) were not guarded. c) Warehouse, Building #351 - On or about 05/21/19 and at times prior thereto, the power feed conveyor (inlet), the blade, and the outlet section of the conveyor in the Diehl Rip Saw #5W5013 were not guarded. d) Warehouse, Building #351 - On or about 05/21/19 and at times prior thereto, the guard in the back of the Hendrick programmable saw was removed, exposing mechanical parts in sudden, intermittent rotation and displacement. e) Warehouse, Building #351 - On or about 05/21/19 and at times prior thereto, the punch section of the 42 Ton Uni-Hydro Ironworker #OW4348 was not guarded. f) Warehouse, Building #351 - On or about 05/21/19 and at times prior thereto, the notcher section of the 42 Ton Uni-Hydro Ironworker #OW4348 was not guarded. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. a) Warehouse, Building #351 - On or about 05/21/19 and at times prior thereto, the heavy duty electric switch for the Rip Saw #OW4330 (Mattison Machine Works, Model #404, S/N 23282) was blocked by a metallic rack, and was not readily accessible in case of service or emergency. b) Warehouse, Building #380 - On or about 05/21/19 and at times prior thereto, the metallic pallet racks were situated at a 20 in. distance from the Main Panel B #003786. (Ref: 1910 Subpart S, Table S-1). In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
