29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about May 23, 2019, and at times prior thereto, the employer allowed employees to operate and work around machines that were not guarded:  1) At the East Grinder, employees used a stairway to access the top of a grinding machine where there was an opening with rotating parts that was not guarded.  This exposed the employees to amputation hazards.   2) At the East Grinder, the grinder port contained moving parts and was not guarded. This exposed the employees to amputation hazards.
