29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a)  On May 31, 2019 at the address of 2305 Whispering Pine Drive in Toledo, OH: The employer did not have a plan or program to identify job site hazards which assist employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)       On May 31, 2019, at the address of 2305 Whispering Pines Drive in Toledo, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing installation.  Employees were exposed to approximately a 9 foot fall hazard from the roof to the ground below.

During this activity, employees were exposed to falls up to 9ft. to the ground below.
