One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about May 27, 2019, the employer did not ensure one or more methods of machine guarding was provided to protect employees from the line #8 reciprocating conveyor point of operation while working on or near the reciprocating conveyor to remove pieces of bread dough.

29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. On or about May 27, 2019, the employer did not ensure employees hands were not placed in the danger zone (point of operation) of the line #8 reciprocating conveyor when using a hand tool to remove dough from the conveyor.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about May 27, 2019, the employer did not ensure an employee cleaning the line #8 reciprocating conveyor performed and followed the established energy control procedures by; ensured the machine was turned off [per 1910.147(d)(2) requirements], isolated the machine from the energy source(s) [(d)(3)], affixed a lockout device [(d)(4)(i)], and verified de-energization and isolation had been accomplished prior to cleaning the machine [(d)(6)].
