29 CFR 1910.28(b)(1)(i): Unprotected sides and edges. Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: 1910.28(b)(1)(i)(A) Guardrail systems; 1910.28(b)(1)(i)(B) Safety net systems; or 1910.28(b)(1)(i)(C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.  a) On or about April 15, 2024, maintenance employees lighting the burners on the Baker Perkins direct gas fired oven (oven #8) in the Roll Line area were exposed to a fall hazard of six-foot and nine-inches (6'9"); the employer did not ensure that each employee walking and working on top of oven #8 with unprotected sides and edges that are six-foot and nine inches (6'9") above the lower level, are protected from falling by one or more of the following: 1910.28(b)(1)(i)(A) Guardrail systems; 1910.28(b)(1)(i)(B) Safety net systems; or 1910.28(b)(1)(i)(C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.

29 CFR 1910.263(l)(9)(iii)(b): Ovens with multiple burners shall be equipped with individual atmospheric pilot lights where there is sufficient secondary air in the baking chamber and where gas is available; or else each burner shall be equipped with an electric spark-type ignition device.   a) On or about April 15, 2024, maintenance employees lighting the burners on the Baker Perkins direct gas fired oven (oven #8) in the Roll Line area were exposed electrical shock hazards of two times five thousand volts (2 x 5000 V) at twenty-two milliamperes (22mA); the employer did not ensure all electric spark-type ignition devices were properly maintained and kept in working order requiring employees to manually light the burners.

29 CFR 1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee did not use insulated tools or handling equipment when the tools or handling equipment might have made contact with such conductors or parts:  a) On or about April 15, 2024, maintenance employees lighting the burners on the Baker Perkins direct gas fired oven (oven #8) in the Roll Line area were exposed electrical shock hazards of two times five thousand volts (2 x 5000 V) at twenty-two milliamperes (22mA); the employer did not ensure each maintenance employee used an insulated tool and/or handling equipment (wand and torch) when the wand and torch made contact with output side of the C10-LF3 transformer.
