29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a)115 N Rt 130, Bordentown, NJ: on or about June 5, 2019 four New View Construction LLC employees were working from a cantilever section of an EZ Scaffold mast climbing scaffold while installing brick fascia on a commercial building. The employer did not ensure that the scaffold had been inspected for defects prior to each work shift. As such, several improperly installed components were not discovered, resulting in the collapse of the cantilever section and the injury of four workers. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration and such activities were not performed by only experienced and trained employees: a)115 N Rt 130, Bordentown, NJ: on or about June 5, 2019 four New View Construction LLC employees were working from a cantilever section of an EZ Scaffold mast climbing scaffold while installing brick fascia on a commercial building. The employer did not ensure that the scaffold had been erected under the supervision of an individual knowledgeable in the manufacturer's Safety & Installation manual as well as the applicable scaffold construction standards. As such, several components were improperly installed, resulting in the collapse of the cantilever section and the injury of four workers. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
