29 CFR 1904.4(a): Each employer required by this part to keep records of fatalities, injuries, and illnesses did not record each fatality, injury and illness that was work-related, a new case, and that meets one or more of the general recording criteria of �1904.7 or the application to specific cases of ��1904.8 through 1904.12: LSC Communications, 216 Greenfield Road, Lancaster, Pennsylvania - On or about June 11, 2019, the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for calendar year 2019: (a) On or about February 22, 2019, a roll tender in the LGM Offset Pressroom sustained a contusion and abrasion to the head after a falling deck in Unit 4 struck this employee's head, and a physician or other licensed health care professional gave the employee two days away from work. (b) On or about April 1, 2019, a bindery material handler in the LGE Bindery developed left wrist pain with decreased range of motion after doing one-handed work using only the left hand due to an on-going work restriction to not use the right hand after a strain/sprain to the right wrist on or about December 11, 2018. A physician or other licensed health care professional then restricted the employee to no use of the left hand, resulting in days away from work from April 2, 2019 to the present time since the employee became restricted from using both hands. Corrected during the inspection. No abatement certification or documentation required.
