29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces. Facility: The employer did not ensure through providing fixed and secured stairway so that employees were able to safely access and/or egress from the elevated working platform (platform). On or about June 11, 2019, the stairway shifted while an employee was stepping onto the top step/tread of the stairway from the platform causing the foot got caught in between the stairway and platform, and the employee sustained minor laceration to the head.
