29 CFR 1904.4(a): Each employer required by this part to keep records of fatalities, injuries, and illnesses did not record each fatality, injury and illness that was work-related, a new case, and that meets one or more of the general recording criteria of �1904.7 or the application to specific cases of ��1904.8 through 1904.12: LSC Communications, 1375 Harrisburg Pike, Lancaster, Pennsylvania - On or about June 11, 2019, the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for calendar year 2019: (a) On or about February 21, 2019, a material handler in the LCE Bindery sustained a back injury after lifting skids that had stuck together, and a physician or other licensed health care professional gave this employee 13 days of restricted work activity. (b) On or about May 14, 2019, a material handler in the LCE Bindery sustained a strain/sprain to the left wrist/thumb while gripping and lifting paper stock off a skid, and a physician or other licensed health care professional gave this employee 23 days of restricted work activity. (c) On or about May 30, 2019, a press material handler in LCM sustained a right knee injury from running into a skid, and a physician or other licensed health care professional gave this employee at least 12 days of restricted work activity. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
