29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) 504 Queen Ann Circle Lot 10: On June 12, 2019 and times prior thereto the employer exposed employees to struck-by hazards from pneumatic nail guns at approximately 120 psi that they were using to deck the roof of a new residential construction without wearing eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 504 Queen Ann Circle Lot 10: On June 12, 2019 and times prior thereto the employer did not ensure approved fall protection system were used by employees while decking the roof on a new residential construction exposing employees to fall hazards of 8 feet 3 inches to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) 504 Queen Ann Circle Lot 10: On June 12, 2019 and times prior thereto the employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Employees were exposed to fall hazards of 8 feet 3 inches to the ground below while installing decking boards on a new residential construction without using fall protection.
