(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: a. On or before June 20, 2019, in the wastewater treatment plant, the employer did not ensure that employees who performed activities associated with the operation of the Perrin filter press (model: 100-S-48; S/N: 14904) on the platform were protected from falls of approximately 7 feet, 4.5 inches into the bed of a dump truck below.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires. a. On or before June 14, 2019, in the Cupola fan building, the employer did not ensure that the horizontal shaft for the Cupola fan was completely guarded, thereby exposing employees to a caught-in and/or contact hazard.
