29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: GCC, Bldg. 300: A Velocity harness used by employees for fall protection during roof demolition at heights up to 13 ft. was not taken out of service when the webbing was frayed, and the manufacturers label was degraded and illegible. Damaged fall protection components may fail in the event of an impact load.
