29 CFR 1910.303(b)(1)(iv): Electrical equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment was not determined using the following considerations: Electrical insulation; (a) Facility, 600 Mayer Street, Conditioning Department, Bridgeville, PA: On or about June 19, 2019 duct tape was used to repair damage to electrical insulation on wiring from the junction box to the motor on the Midwest #4 and #5 grinders and the employer did could not ensure that the tape maintained the necessary insulating factor.
