29 CFR 1910.178(o)(2): Load(s) were being handled which were outside the rated capacity of the industrial truck(s): (a) Facility, Scrap Yard/Hot Mill: On or about July 26, 2025, the Taylor forklift was used to push the loaded scrap transfer car (35 to 40 tons) from the Scrap Yard to the Hot Mill and then back again. This scrap transfer car sits on rails and has been pushed by 2 different forklifts for 2-3 years.

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made. (a) Facility, Scrap Yard/Hot Mill Area: On or about July 26, 2025, approximately 25% of the time (especially on 2nd shift) there was no spotter or other mechanism to warn employees that the scrap car was being pushed by a powered Industrial truck to and from the Scrap Yard and Hot Mill. This included being pushed across a roadway.

The trolly brakes worked intermittently. 2. The bridge brakes worked intermittently. (b) On or about January 7, 2026, the #2 Hot Metal Crane in the Hot Metal Shop had the following had the following problem: 1. The bridge brakes worked about 5% of the time. (c) Facility, Labor Crane: On or about January 7, 2026, the bridge brakes to the Labor Crane were not working properly.
