The operator of the Milwaukee vertical mill would use the compressed air at least once every day to clean metal chips from the Milwaukee vertical mill.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility: On or about December 2, 2022, an employee was operating a Milwaukee vertical milling machine and there was no guarding nor device to keep the operator from contacting the rotating cutting head of the machine. In addition, the employee was using a no touch tool that was about a foot long to remove chips from this machine while the machine was running.

29 CFR 1910.212(a)(3)(iii):Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: (a) Facility: On or about December 2, 2022, an employee was using a no touch tool (15 inch small rake) to remove chips from the Milwaukee vertical mill. The metal rake fingers got caught in the rotating cutters of the vertical milling machine and pulled the employees hand towards the cutters. The operator suffered a partial amputation of his right thumb.
