29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: a) Jobsite, PI 19008 Washinko Road, Latrobe, PA: On or about June 19, 2019, two employees were working on a residential roof and the owner told the csho that the employees had been trained in fall protection, but did not produce any written documentation demonstrating that.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: a) Jobsite, PI 19008 Washinko Road, Latrobe, PA: On or about June 19, 2019, two employees were working on a residential roof and were utilizing fall protection, but had their lanyards attached to the D ring in the front of their harness. The owner told the csho that the employees had been trained in fall protection.
