On or about January 29, 2024, on the northwest side of the building, an employee performing painting work activities was observed working from the second level of a scaffold that was not fully planked.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. On or about January 29, 2024, employees performing painting work activities used walk ways that were not at least 18 inches wide: a.) A walk way, used to access the metal roof, was erected on the scaffold located on the west side of the building. b.) A walkway, used to access the metal roof, was erected on the scaffold located on the east side of the building. c.) A walk way, used to access the main roof, was erected on the scaffold located on the east side of the building.
