Powered industrial truck operators shall receive initial training in the following topics: (ii)(E): Narrow aisles and other restricted places where the vehicle will be operated: (ii)(I): Workplace-related topics: Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation. a) On or about June 25, 2019, Yamada North America, Inc., did not ensure that all employees received training on the unique conditions in the work place which could effect safe operations of the powered industrial truck in that, forklift operations and pedestrians are operating inside of the OPI trailer with a 14-degree incline exposing the pedestrian to a stuck by/crushing hazard.
