29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 06/26/19, on a roofing jobsite, at a house at 360 Ashland Ave., Buffalo, NY: A Ladder Jack Scaffold was erected on the left, back side of the house. An employee was on the scaffold while installing ice shielding on the roof. The employee was wearing a full body harness, but was not tied off to anything. There were no roof anchors/lifelines installed in the area where the employee was working. The employee was exposed to a fall of approx. 18 ft. NO ABATEMENT CERTIFICATION REQUIRED
