29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 11/04/19, on a roofing job site, at a house at 84 Norwood Ave., Buffalo, NY: A ladder jack scaffold was erected on the north side of the house. Two employees were on the scaffolding, cleaning the edges and putting in a fascia board. The employees were exposed to a fall of approx. 20 ft. because they were not tethered to the lifelines that were provided. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another: a) On or about 11/04/19, on a roofing job site, at a house at 84 Norwood Ave., Buffalo, NY: A ladder jack scaffold consisting of 3 support ladders, two 16 ft. long, 12 inch wide aluminum piks, and a separate access ladder was erected on the north side of the house. The pik on the east end of the scaffold overlapped (bridged) the pik, which was on the west end of the scaffold by about 3 ft. at the middle support ladder. Two employees were on the scaffolding cleaning the edges and putting in a fascia board. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 11/04/19, on the roofing jobsite at a house at 84 Norwood Ave., Buffalo, NY: An extension ladder was set up on the ground and used by employees to access the aluminum piks on the ladder jack scaffold set up on the north side of the house. The siderails of that extension ladder were only approx. 20 inches above the pik. NO ABATEMENT CERTIFICATION REQUIRED
