29 CFR 1926.453(b)(2)(v): A body belt with lanyard in a fall restrain system or a fall arrest harness attached to the boom or basket was not worn by employee(s) working from an aerial lift: a. On or about 06/20/2019, at the front of the building, an employee was exposed to 13 foot 10 inches fall hazard, in that, a JLG 400S aerial lift was used to gain access to the roof without any type of fall protection. John Michael Miller Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.453(b)(2)(v) which was contained in OSHA inspection number 1073021 citation number 1, item number 4 and was affirmed as a final order on 07/22/2015, with respect to a workplace located at 6476 Strip Ave NW, Canton, OH.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. On or about 06/20/2019, at the front roof area over the entrance to the building, an employee working on a 7.5 to 12 pitch roof was exposed to a 13 foot 10 inches fall hazard while building a dormer window on the roof of the building, without utilizing any type of fall protection. John Michael Miller Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(1), which was contained in OSHA inspection number 1299907 citation number 1, item number 1 and was affirmed as a final order on 08/01/2018, with respect to a workplace located at 25830 Sierra Center Blvd, Lutz, FL.
