29 CFR 1910.179(n)(3)(ii)(c): The employer failed to ensure that the lift hook was brought over the load prior to lifting in such a manner as to prevent swinging:  (a)Gage Brothers Concrete Products, Inc. at 2800 North Bahnson Avenue, Sioux Falls, SD 57104: On or about and at times prior to July 13, 2019, the employer did not ensure the gantry crane operator had the lifting hook centered over the precast concrete panel prior to lifting the panel.  An employee entered the hazard zone between the two panels to reposition dunnage used to protect the panel when the panel became unstable and swung, striking the employee and crushing him between the two panels. This condition exposed employees to struck-by and crushing injuries.    Abatement note:    1. Employer should ensure that gantry crane operators remain outside the hazard zone, able to visually observe all activities related to proper hoisting and moving of precast panels.   2. The gantry crane operator shall not leave their position at the controls while the load is suspended.  This includes the operator performing job functions such as wedge placement that put them directly in the hazardous area of operation. 3. Employer should ensure that all temporary and permanent employees are trained to understand the hazards associated with precast panel movement and gantry crane operations.
