29 CFR 1910.213(c)(1):Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: (a)Gage Brothers Concrete Products, Inc at 2810 North Bahnson Avenue in Sioux Falls, SD 57104: On or about and at times prior to May 21, 2021, the employer did not ensure employees were protected from amputation hazards in that the SawStop 10" Industrial Cabinet Saw (Model: ICS31230 S/N:1141700831) was operated without a guard when ripping 2"x 4" lumber. An employee suffered a fingertip amputation after contacting the unguarded blade. This condition exposed employees to amputation hazards.

29 CFR 1910.213(c)(3):Hand-fed ripsaw(s) did not have non-kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator: (a)Gage Brothers Concrete Products, Inc at 2810 North Bahnson Avenue in Sioux Falls, SD 57104: On or about and at times prior to May 21, 2021, the employer did not ensure employees were protected from struck-by hazards in that the SawStop 10" Industrial Cabinet Saw (Model: ICS31230 S/N:1141700831) was operated without anti-kickback device(s) when ripping 2"x4" lumber. An employee suffered a fingertip amputation after stock became caught and kicked back causing the operators hand to contact the unguarded blade. This condition exposed employees to struck-by hazards.
