29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) 1576 Spring Valley Road State College - An employees was utilizing a JLG 450- AJ aerial lift to access the side wall of a building foundation and was not protected/restrained inside the basket with a fall protection harness and lanyard, exposing the employees to a possible ejection and fall hazard of approximately 15 feet, on or about July 10, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
