Samya Company LLC; W Main St, Grove City, PA 16127: Northwest side of the building- On or about 12-30-2024 and at times prior thereto; employee was working out of a JLG E300AJP aerial lift (serial #0300110367) without the use of fall protection. Employee fell 20-feet to ground level.
