Samya Company LLC; W Main St, Grove City, PA 16127: Northwest Mezzanine- On or about 01-02-2025 and at times prior thereto; Employees perform work on the Northwest Mezzanine with an incomplete guardrail at the ladder access area. Employees are exposed to a 10-foot fall hazard while accessing the mezzanine to work on the heaters for the building.
