Berkeley Lake Road NW, Duluth, GA (Warehouse) - An employee (temporary) were exposed to struck-by hazard.  The employer did not provide the proper powered industrial training, prior to letting the temporary worker operates the sit-down Toyota forklift, on or about July 5, 2019.
