29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. The Presidio Trust: An effective hearing conservation program was not formulated and administered for employees who were exposed to noise level at or above 85 decibels on an 8-hour TWA.

29 CFR 1904.32(b)(1): The employer did not review the OSHA 300 Logs entries as extensively as necessary to make sure that they are complete and correct. The Presidio Trust: The hearing loss incidents that were recorded on OSHA 300 Logs did not occur in 2018.
