29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed:  Masonry shop: On and/or before 3/5/2024, employees used a Delta radial arm saw to cut wood. The sides of the lower exposed portion of the blade were not guarded. Employees were potentially exposed to contact with rotating saw blades which can cause amputation injuries.

29 CFR 1910.303(b)(7)(iv): Electric equipment was not free from damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.   Outside the water treatment plant: On and/or before 3/5/2024, employees were exposed to potential electrical hazards from an inoperable General Electric safety disconnect switch. The handle was broken and detached from the safety disconnect switch.  This condition potentially exposed employees to electrical shock, and/or burn hazards.

29 CFR 1910.334(a)(2)(ii): When there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe had been made:  In the electric shop:  A damaged power cord was supplying power to a DeWalt Arbor saw. The outer insulating sleeve had slipped off near the plug exposing the inner wiring insulation. This condition potentially exposed employees to electric shock.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are barrier guards, two-hand tripping devices, electronic safety devices, etc.  Building 1340: On and/or before 3/5/2024, employees used a bench grinder for masonry tasks. The Dayton 8" bench grinder did not have a guard provided. This condition potentially exposed employees to eye, laceration, and/or amputation hazards.  The Presidio Trust was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1466118, citation number 1, item number 4 and was affirmed as a final order on 04/10/2020, with respect to a workplace located at 1750 Lincoln Blvd, San Francisco, CA 94129.

29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving:  (a) Building 1340: On and/or before 3/5/2024, employees used a DeWalt DW 758 bench grinder which was not secured to the work bench to prevent walking or moving. This condition potentially exposed employees to abrasions.    (b) Building 1063:   On and/or before 3/5/2024, employees used a Makita miter saw, model LS 1040, which was not secured to the work bench to prevent movement. This condition potentially exposed employees to lacerations.  The Presidio Trust was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(b) which was contained in OSHA inspection number 1466118, citation number 1, item number 5 and was affirmed as a final order on 04/10/2020, with respect to a workplace located at 1750 Lincoln Blvd, San Francisco, CA 94129.

29 CFR 1910.266(d)(2)(iv): The employer did not maintain the contents of the first aid kit in serviceable condition:   Presidio worker transport truck used by the forestry department: On and/or before 3/5/2024, the first aid kit was not maintained in a serviceable condition. Employees work in outdoor conditions where they may need to use a first aid kit. Items including antimicrobial hand sanitizing wipes and the contents of an insect sting kit were expired. This condition potentially exposed employees to reduced efficacy of first aid kit medicines.

29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  IT Building: On and/or before 3/6/2024: The working space around the LP electrical panel was not provided and maintained with a 36 inch clearance. The panel was blocked by phones, printers, keyboards and other equipment. In the event of an emergency in which a circuit needs to be immediately shut down, employees would be unable to quickly access the panel. This condition potentially exposed employees to electrical and/or fire hazards.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  IT Building:  On and/or before 3/6/2024, electrical fittings were not provided with covers approved for the purpose Two light switches did not have covers. Use of light switches without covers can potentially expose employees to electrical hazards from contact with energized electrical parts.
