Employees were required to work on the equipment with procedures that were generic, vague, and did not list 1) types and locations of machine or equipment operating controls; 2) types and locations of energy isolation devices; 3) and sequence or steps to perform the lockout.

Employees were required to work on machines with procedures that had not been audited periodically as required by the standard so that any deviations or inadequacies would be discovered and corrected.
