29 CFR 1926.351(b)(2): Welding cable(s) in use were not free from improper repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected:  a)    Fort Belvoir: On or about 07/24/2019, an employee used a welding lead to weld angle irons on the exterior of a stair tower that had cuts through the outer jacket at 33 inches and 88 inches from the electrode holder, as a result employee was exposed to a possible shock hazard.   No abatement certification or documentation required.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection:  a)     Fort Belvoir: On or about 07/24/2019, a Metabo portable grinder powered by a Trailer Blazer 325 diesel, DC welder 12,000 Watt generator was that did not have a ground fault circuit interrupter device as a result an employee was exposed to a possible shock hazard.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access:  a)    Fort Belvoir: On or about 07/24/2019, employees used an 8ft detached stepladder to access and egress a tubular welded frame scaffold erected approximately 7 feet from the deck, as a result was exposed to a possible fall hazard.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
