29 CFR 1926.501(b)(2)(ii): Each employee on a walking/working surface 6 feet (1.8 m) or more above a lower level where leading edges are under construction, but who was not engaged in the leading edge work, was not protected from falling by a guardrail system, safety net system, or personal fall arrest system.  (a) On or about July 26, 2019, at 4142 Bluebonnet Blvd., employees shingled the roof of an office building without adequate fall protection at heights of approximately 12 and 18 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about July 26, 2019 at 4142 Bluebonnet Blvd. building site the employees were using an extension ladder to access the roof without the ladder being extended at least 3 feet beyond the roofline.
