29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.  On or about February 27, 2020 employees on a steep roof were not protected from falling by a fall arrest system. Employees were installing a sheet metal roof on the 150' X 140' roof of the gymnasium that had a 3:12 pitch.  Premiere South, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(2)(ii), which was contained in OSHA inspection number 1418442, citation number 1, item number 1 and was affirmed as a final order on November 28, 2019, with respect to a workplace located at 4740 Bluebonnet Blvd, Baton Rouge, LA.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 27, 2020 at 19010 Ponchatoula Parks Drive, Ponchatoula, LA employee were allowed to use ladders for access and egress to/from the upper levels of the structure were  not extended at least 3 feet above the landing.   The Premiere South, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1418442, citation number 2, item number 1 and was affirmed as a final order on November 28, 2019, with respect to a workplace located at 4740 Bluebonnet Blvd, Baton Rouge, LA.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   On or about February 27, 2020, at Premier South LLC, employees that were exposed to fall hazards did not receive a written (training) certification record.
