29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 22, 2021, on the south side of the project site, an employee was observed performing debris removal work without proper head protection under potential falling objects from heights upwards of fifteen (15) feet.

29 CFR  1926.602(c)(1)(vi):Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.  On or about April 22, 2021, on the south side of the project site, an employee was observed performing debris removal work while under an extended telescopic forklift, loaded with debris at heights upwards of fifteen (15) feet.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 22, 2021, on the south side of the project site, employees were observed on a roof without proper fall protection at heights upwards of fifteen (15) feet.  Premier South LLC was previously cited for a repeat violation of an equivalent standard 29 CFR 1926.501 (b)(10), which was contained in OSHA inspection number 1466238, citation number 1, item number 1 and was affirmed as a final order on May 01, 2020, with respect to a workplace located at 19010 Ponchatoula Parks Dr., Ponchatoula, LA, 70454.  Premier South LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(2)(ii), which was contained in OSHA inspection number 1418442, citation number 1, item number 1 and was affirmed as a final order on November 28, 2019, with respect to a workplace located at 4740 Bluebonnet Blvd, Baton Rouge, LA.
